SWAN Primary Care Network

South Wigan Ashton North (SWAN) and Wigan Central Primary Care Networks (PCNs) are collaboratively delivering this health promotion session, in partnership with Be Well, Work Well and Community Link Worker Teams from Wigan Borough Council.

Primary Care Networks (PCNs) are groups of GP practices, working together with other health and social care teams and services, to meet the health and social needs of their local populations.

SWAN Primary Care Network: Kumar Family Practice, South Wigan Medical Practice, Marus Bridge Practice, The Grange Practice, Winstanley Medical Centre, Bryn Cross Surgery and The Medicentre.
